ABSTRACT

Introducción: La elaboración de comidas caseras implica pérdidas de vitaminas según el procedimiento de cocción. El uso de alimentos reforzados ayudaría a cubrir las recomendaciones nutricionales durante periodos críticos de la vida. El objetivo de este estudio piloto experimental es analizar la variación de las concentraciones de retinol y ácido ascórbico, en preparaciones reforzadas que incluyeron un cereal fortificado y analizar su valor como estrategia alimentaria. Material y métodos: El retinol y el ácido ascórbico se seleccionaron como indicadores de labilidad para tres recetas con diferentes procedimientos de cocción elaboradas de manera casera en condiciones reproducibles. El retinol se analizó mediante HPLC y el ácido ascórbico mediante electroforesis capilar en una muestra y HPLC en otras, debido a los límites de detección de cada método. Resultados: Según el método, el retinol aumentó su concentración en las muestras post-cocción debido a la pérdida de agua por evaporación (14% entre las reforzadas) o la redujo debido a las condiciones de temperatura, pH o presencia de oxígeno por cocciones a horno o fuego directo (5% y 22% respectivamente). Las muestras reforzadas cocidas presentaron un aumento del 18% - 20% frente a las no reforzadas. La cocción por conducción (sartén) u horno (corrientes de convección por aire) generó una pérdida parcial (13% y 24% respectivamente) de ácido ascórbico. La pérdida fue total en cocción a fuego directo y mayor relación superficie/volumen. Las muestras reforzadas cocidas presentaron un aumento de 90% cuando la vitamina se conservó. Las variaciones finales de concentración estuvieron relacionadas con las concentraciones iniciales y los métodos de cocción con diferentes formas de transferencia de calor y presiones parciales de O2. Conclusiones: Las vitaminas estudiadas son inestables en las comidas tras la cocción doméstica, aunque su pérdida es parcial; por lo tanto, reforzar las preparaciones con productos alimentarios reforzados sería una estrategia útil para cubrir los micronutrientes en situaciones críticas, en sistemas con baja densidad nutricional y diferentes consistencias


Introduction: Homemade meals elaboration implies vitamins losses in different cooking procedures. Using reinforced food would cover nutrient recommendations during critical periods of life. The aim of this experimental pilot study was to evaluate the variation of ascorbic acid and retinol concentrations in reinforced meals with fortified cereals and to analyze their value as a feeding strategy. Materials and Methods: These vitamins were selected as indicators of lability in three recipes with different cooking procedures prepared at home under reproducible conditions. Retinol was analysed through HPLC and Ascorbic Acid by capillary electrophoresis in one sample and HPLC in others because of detection limit of each methods. Results: According to cooking method, Retinol increased its concentration in post-cooking samples due to water evaporation (14% among enriched) or reduced it due to temperature, pH or presence of oxygen by oven or direct cooking (5% and 22% respectively). Between cooked samples, reinforced sample presented an 18% - 20% increase versus non-enriched one. Cooking by conduction (frying pan) or oven (air convection currents) generated a partial loss (13% and 24% respectively) of ascorbic acid. The loss was total in direct fire firing and higher surface/volume ratio. Cooked reinforced samples showed a 90% increase when the vitamin was preserved. Final concentration variations were related to initial concentrations and cooking methods involving different forms of heat transfer and partial pressures of O2.Conclusions: Vitamins studied were shown to be unstable in meals after domestic cooking, although their loss is partial. Therefore,reinforcing preparations with fortified food products would be a useful strategy to cover micronutrients in critical situations, in food preparations with low nutritional density and different consistencies

ABSTRACT

Objective:To investigate the clinical efficacy of vitamin D drops combined with insulin aspart in the treatment of gestational diabetes mellitus (GDM), and the effect of vitamin D drops on the serum levels of 1, 25 dihydroxyvitamin D 3 [1, 25(OH) 2D 3] and retinol binding protein 4 (RBP4). Methods:A total of 94 GDM patients admitted to the Baoding Second Central Hospital from March 2019 to March 2021 were selected and randomly divided into an observation group and a control group with 47 cases each using a random number table method. The control group received subcutaneous injection of insulin aspartate for treatment, while the observation group received oral vitamin D drops for treatment. After 4 weeks of continuous treatment, the blood glucose control effect and adverse reactions were observed in both groups. The glucose metabolism indicators of the two groups were compared before and after treatment, including fasting blood glucose (FPG), 2-hour postprandial blood glucose (2-hour PG), insulin resistance index (HOMA-IR), and pancreatic islets β Cell Function Index (HOMA-β) and serum levels of 1, 25(OH) 2D 3, RBP4, lipoprotein related phospholipase A2 (Lp-PLA2), and vascular cell adhesion molecule-1 (VCAM-1). All patients were followed up until the end of pregnancy, and Statistical analysis was conducted on the adverse outcomes of two groups of mothers and infants. Results:The time to reach the standard for FPG and 2-hour PG in the observation group, as well as the time for both to reach the standard were significantly shorter than those in the control group (all P<0.05). There was no statistically significant difference in the incidence of dawn phenomenon and hypoglycemia between the observation group and the control group (all P>0.05). After treatment, FPG and 2-hour PG in both groups were significantly reduced compared to those before treatment (all P<0.05); However, after treatment, there was no statistically significant difference between the groups (all P>0.05). Compared with before treatment, HOMA-IR in both groups significantly decreased (all P<0.05), All HOMA- β significantly increased (all P<0.05); And the improvement was more significant in the observation group (all P<0.05). After treatment, the serum levels of 1, 25(OH) 2D 3 in the observation group significantly increased compared to that before treatment ( P<0.05), but there was no significant change in the control group before and after treatment ( P>0.05). After treatment, the levels of serum RBP4, Lp-PLA2, and VCAM-1 in both groups significantly decreased compared to those before treatment (all P<0.05); After treatment, the serum levels of RBP4, Lp-PLA2, and VCAM-1 in the observation group were lower than those in the control group (all P<0.05). The incidence of adverse maternal and neonatal outcomes in the observation group was 14.9%(7/47) and 10.6%(5/47), respectively, which were lower than those in the control group [34.0%(16/47) and 27.7%(13/47)] (all P<0.05). There were 8 cases of hypoglycemia in 94 patients (3 in the observation group and 5 in the control group), and no other adverse events occurred. Conclusions:The combination of vitamin D drops and insulin aspartate in the treatment of GDM can safely, effectively, quickly, and steadily control patients′ blood sugar, improve IR and pancreatic islets β The effect of cell function on reducing the incidence of adverse maternal and fetal outcomes may be related to increasing serum 1, 25(OH) 2D 3 levels and down-regulating the expression levels of serum RBP4, Lp-PLA2, and VCAM-1.

ABSTRACT

Objective:To explore the relationship between serum retinol binding protein(RBP)and metabolic-associated fatty liver disease(MAFLD)in elderly patients with type 2 diabetes mellitus(T2DM)and possible underlying metabolic mechanisms.Methods:A total of 3384 elderly T2DM patients hospitalized and with complete clinical records at the Department of Endocrinology and Metabolism, Sixth People's Hospital Affiliated to Shanghai Jiao Tong University between January 2003 and December 2012 were recruited in this retrospective study.Patients were divided into four groups according to the quartiles of serum RBP levels: the first quartile of serum RBP levels(<35 mg/L, 844 cases), the second quartile of serum RBP levels(35 mg/L≤ RBP ≤41 mg/L, 773 cases), the third quartile of serum RBP levels(42 mg/L≤ RBP ≤51 mg/L, 902 cases), and the fourth quartile of serum RBP levels(RBP>51 mg/L, 865 cases). Clinical data and laboratory test results were collected.Differences in the prevalence of MAFLD were compared between the four groups.The association between RBP and MAFLD was analyzed via binary logistic regression.Results:After adjusting for age and sex, the proportion of obesity( χ2=15.222, P<0.01), the percentage using lipid-lowering drugs( χ2=88.552, P<0.01), systolic blood pressure( F=12.002, P<0.01), diastolic blood pressure( F=6.872, P<0.01), waist circumference( F=9.563, P<0.01), waist-hip ratio( F=7.972, P<0.01), body mass index( F=9.057, P<0.01), serum creatinine( χ2=185.445, P<0.01), serum uric acid( χ2=314.691, P<0.01), 24-hour urinary albumin( χ2=91.012, P<0.01), alanine aminotransferase( χ2=17.049, P=0.003), γ-glutamyl transpeptidase( χ2=50.514, P<0.01), total cholesterol( F=45.669, P<0.01), triglycerides( χ2=361.269, P<0.01), low-density lipoprotein( F=8.772, P<0.01), fasting C-peptide( χ2=165.756, P<0.01), 2h postprandial C-peptide( χ2=120.690, P<0.01), and the homeostasis model assessment of insulin resistance(HOMA2-IR)( χ2=148.884, P<0.01)in elderly patients with T2DM all showed a clear upward trend.The prevalence of MAFLD also gradually increased across the quartiles of serum RBP levels[26.5%(224/844), 30.1%(233/773), 36.6%(330/902), and 41.8%(362/865)], respectively( χ2=52.526, P<0.01). Elderly T2DM patients with MAFLD had a significantly higher value of HOMA2-IR than those without MAFLD[2.0(1.31-2.8) vs.1.39(0.86-2.06), F=220.826, P<0.01]. After correcting for other confounding factors, binary logistic regression showed that serum RBP was strongly associated with the presence of MAFLD in elderly patients with T2DM( β=0.209, 95% CI: 1.079-1.408, OR=1.232, χ2=9.441, P<0.01). Conclusions:Elevated serum RBP levels are an independent risk factor for the development of MAFLD in elderly T2DM patients, possibly through increased insulin resistance induced by RBP.

ABSTRACT

Objective:To investigate the relationship between the expressions of serum trans thyroxine protein (TTR) , retinol binding protein 4 (RBP4) and metabolic syndrome of pregnancy (GMS) .Methods:A total of 103 patients with GMS from May. 2017 to Dec. 2019 were selected as the observation group, in addition, 100 healthy pregnant women with single pregnancy and no complications were selected as control group. Before pregnancy, the height and weight were detected. After the diagnosis of GMS, serum markers and blood pressure were detected. The levels of total cholesterol (TC) , triglyceride (TG) , low density lipoprotein cholesterol (LDL-C) , high density lipoprotein cholesterol (HDL-C) and fasting blood glucose (FBG) were detected by Hitachi 7600 automatic biochemical analyzer, and the expression levels of TTR and RBP4 in serum were detected by enzyme-linked immunosorbent assay (ELISA) . Receiver operating characteristic (ROC) curve was drawn to evaluate the diagnostic value of TTR and RBP4 in GMS. Multivariate Logistic regression model was used to analyze the influencing factors of GMS in pregnant women.Results:There was no significant difference in age or gestational weeks between the two groups ( P>0.05) . Compared with those in the control group, the levels of BMI, TC, TG, LDL-C, FBG, systolic blood pressure, diastolic blood pressure, serum TTR and RBP4 expression levels were higher in the observation group before pregnancy ( P<0.05) , while HDL-C was lower ( P<0.05) . The area under the curve (AUC) of TTR and RBP4 alone or combination in the diagnosis of GMS was 0.797, 0.816 and 0.898, respectively. The cut-off value of TTR was 284.91 mg/L, and the sensitivity and specificity were 63.10% and 89.00%, respectively. The cut-off value of RBP4 was 17.89 mg/L, and the sensitivity and specificity were 69.90% and 87.00%, respectively. The sensitivity and specificity of the combined diagnosis were 83.50% and 84.00% respectively. Multivariate Logistic regression analysis showed that high level of TTR and high level of RBP4 were independent risk factors of GMS in pregnant women ( P<0.05) . Conclusions:The expression levels of serum TTR and RBP4 in GMS patients are high, and they have certain diagnostic value for GMS. They are independent risk factors of GMS in pregnant women, which may provide new ideas for the early diagnosis and prevention of GMS.

ABSTRACT

AIM: To explore the efficacy of preoperative intravitreal injection of conbercept combined with 25G+ pars plana vitrectomy(PPV)in the treatment of proliferative diabetic retinopathy(PDR).METHODS: The clinical data of 154 patients(176 eyes)with PDR admitted to our hospital from January 2019 to June 2021 were collected for retrospective analysis. According to the treatment methods, 80 patients(92 eyes)in combined treatment group were treated with preoperative intravitreal injection of conbercept combined with 25G+PPV, and 74 patients(84 eyes)in control group were given 25G+PPV only. The postoperative clinical efficacy and levels of adipokines [adiponectin(APN), retinol binding protein 4(RBP4)] before and after surgery were compared between both groups of patients.RESULTS: The combined treatment group showed better clinical efficacy than the control group at 1mo after surgery(P&#x003C;0.05). Both groups had lower RBP4 levels at 3mo after surgery(P&#x003C;0.05), with the combined treatment group showing a lower level than the control group(P&#x003C;0.05). Serum APN levels significantly increased in both groups after surgery(P&#x003C;0.05), with the combined treatment group having a higher level than the control group(P&#x003C;0.05). The combined treatment group had lower incidence rates of retinal proliferation and postoperative complications after than the control group 3mo of follow-up(P&#x003C;0.05).CONCLUSION: Preoperative intravitreal injection of conbercept combined with 25G+PPV is beneficial in improving the therapeutic effect of PDR and reducing the incidence rates of complications, which may be related to the regulations of the expressions of adipokines.

ABSTRACT

Objective: To explore the effect of down-regulation of retinol binding protein 2 (RBP2) expression on the biological characteristics of ovarian cancer cells and its mechanism. Methods: Knockdown of RBP2 and cisplatin (DDP)-resistant ovarian cancer cell line SKOV3/DDP-RBP2i was established, the negative control group and blank control group were also set. Cell counting kit 8 (CCK-8) was used to detect the cell proliferation ability, flow cytometry was used to detect cell apoptosis, scratch test and Transwell invasion test were used to detect cell migration and invasion ability, real-time fluorescent quantitative polymerase chain reaction (RT-qPCR) and western blot were used to detect the expressions of molecular markers related to epithelial-mesenchymal transition (EMT). The effect of RBP2 on the growth of ovarian cancer was verified through experiment of transplanted tumors in nude mice, and the relationships between RBP2 expression and tumor metastasis and patient prognosis were analyzed using the clinical data of ovarian cancer in TCGA database. Results: After down-regulating the expression of RBP2, the proliferation ability of SKOV3/DDP cell was significantly reduced. On the fifth day, the proliferation activities of SKOV3/DDP-RBP2i group, negative control group and blank control group were (56.67±4.16)%, (84.67±3.51) and (87.00±4.00)% respectively, with statistically significant difference (P<0.001). The apoptosis rate of SKOV3/DDP-RBP2i group was (14.19±1.50)%, higher than (8.77±0.75)% of the negative control group and (7.48±0.52)% of the blank control group (P<0.001). The number of invasive cells of SKOV3/DDP-RBP2i group was (55.20±2.39), lower than (82.60±5.18) and (80.80±7.26) of the negative control group and the blank control group, respectively (P<0.001). The scratch healing rate of SKOV3/DDP-RBP2i group was (28.47±2.72)%, lower than (50.58±4.06)% and (48.92±4.63)% of the negative control group and the blank control group, respectively (P<0.001). The mRNA and protein expressions of E-cadherin in the SKOV3/DDP-RBP2i group were higher than those in the negative control group (P=0.015, P<0.001) and the blank control group (P=0.006, P<0.001). The mRNA and protein expression of N-cadherin in SKOV3/DDP-RBP2i group were lower than those in the negative control group (P=0.012, P<0.001) and the blank control group (P=0.005, P<0.001). The mRNA and protein expressions of vimentin in SKOV3/DDP-RBP2i group were also lower than those in the negative control group (P=0.016, P=0.001) and the blank control group (P=0.011, P=0.001). Five weeks after the cells inoculated into the nude mice, the tumor volume of SKOV3/DDP-RBP2i group, negative control group and blank control group were statistically significant different. The tumor volume of SKOV3/DDP-RBP2i group was smaller than those of negative control group and blank control group (P=0.001). Bioinformatics analysis showed that the expression of RBP2 in patients with metastatic ovarian cancer was higher than that without metastasis (P=0.043), and the median overall survival of ovarian cancer patients with high RBP2 expression was 41 months, shorter than 69 months of low RBP2 expression patients (P<0.001). Conclusion: Downregulation of the expression of RBP2 in SKOV3/DDP cells can inhibit cell migration and invasion, and the mechanism may be related to the inhibition of EMT.

ABSTRACT

Objective:To investigate the clinical diagnostic value of joint test of retinol-binding protein (RBP), cystatin C (CYSC) and urinary (albumin/creatinine ratio, ALB/Cr) ALB/Cr in early diabetes nephropathy.Methods:Data of 50 early diabetic nephropathy patients (EDN group) from Jan. 2020 to Jun. 2021 in our hospital, another 50 pure type 2 diabetic patients (T2DM group), and 50 healthy subjects (control group) were compared and analysed. RBP, CYSC and urinary ALB/Cr were tested for the 3 groups. Then the clinical diagnostic value between single index test and joint test for the early diabetes nephropathy were compared.Results:Group EDN had higher RBP, CYSC and urinary ALB/Cr [ (114.66±0.56) mg/L, (2.64±0.33) mg/L, (351.81±15.48) ] mg/g than group T2DM [ (83.58±0.83) mg/L, (1.41±0.29) mg/L, (113.65±12.55) mg/g] and control group[ (38.61±0.66) mg/L, (0.53±0.26) mg/L, (16.36±5.61) mg/g]. The difference was statistically significant ( P<0.05). The specificity and sensitivity of early diabetes nephropathy were 95.38% and 96.21%, both higher than single index test. Conclusion:The combined detection of serum RBP, CYSC and urine ALB/Cr has certain reference value for the clinical diagnosis of early diabetic nephropathy.

ABSTRACT

ObjectiveTo explore the association between dietary retinol intake and prognosis of patients with esophageal squamous cell carcinoma. MethodsThe study enrolled 388 cases with primary esophageal squamous cell carcinoma that was pathologically diagnosed in the first affiliated hospital of Fujian medical university and the cancer hospital of Fujian medical university from July 2014 to August 2019. Chi-square test was used to determine the relationship between retinol and patients' characteristics. Kruskal-Wallis rank sum test was used to analyze the relationship between retinol and multiple nutrients. Cox proportional hazards regression model was used to explore the association between dietary retinol and esophageal squamous cell carcinoma. ResultsPatients with esophageal squamous cell carcinoma did not significantly differ in gender, age, TNM stage, tumor length or nutrient intake by different retinol intakes (P>0.05). Multivariate Cox regression analysis showed that the high retinol intake group had a better prognosis (overal survial:HR=0.279,95%CI:0.150‒0.520,P<0.001;disease-free survival:HR=0.306,95%CI:0.181‒0.516,P<0.001). ConclusionHigh dietary retinol intake may improve the prognosis of patients with esophageal squamous cell carcinoma.

ABSTRACT

Objective:To construct a prokaryotic expression vector for human retinol binding protein 4 (hRBP4) that allows technicians to obtain hRBP4 purified protein with low cost, high efficiency, high concentration and high purity.Methods:The hRBP4 coding sequence provided by National Center for Biotechnology Information was optimized by E. coli codons, and a synthetic DNA fragment was cloned into the PET-28A (+) prokaryotic expression vector to construct a recombinant hRBP4 expression plasmid. The recombinant protein was transformed into E. coli BL21, and the induced expression conditions (temperature, rotate speed and isopropyl β-d-thiogalactoside concentration) were optimized. The recombinant protein was purified by His fusion tag. Results:The recombinant hRBP4 prokaryotic expression plasmid was successfully constructed, and the expression concentration and induction temperature of the recombinant protein were optimized. The results of sodium dodecyl sulfate polyacrylamide gel electrophoresis showed that a band with a relative molecular weight of 26 000 daltons was clearly visible in the purified product. The purified hRBP4 protein could be detected clinically, and there was a good linear relationship between the dilution ratio and the detection concentration.Conclusions:The recombinant hRBP4 protein has high purity, high concentration, and short production cycle. It has the potential to become a candidate for reference materials for laboratory quality evaluations.

Abstract Objective: To describe the current status of micronutrient deficiencies (MD) and anemia in Mexican children. Materials and methods: Serum data from 1 382 pre-school-age children (1-4 years) and 3 590 school-age children (5-11 years) Ensanut 2018-19 participants were analyzed. Iron deficiency (ID), vitamin B12 deficiency (B12D), vitamin A depletion (VADp), and anemia were identified. Logistic regression models were used to identify associations between sociodemographic characteristics of children and MD. Results: Anemia was present in 28.9 and 19.2% of the children aged 1-4 and 5-11, respectively; in ID 10 and 5.1%; B12D, in 5.1 and 4.8%, and VADp, in 4.7 and 4.3%. 18.5% of the preschool-age children and 13% of the school-age children had at least one MD associated to anemia. Conclusions: MD and anemia affect the younger children in larger proportion. Fortification and supplementation programs should be reinforced to avoid the long-term consequences of MD.


Resumen Objetivo: Describir la situación actual de deficiencias de micronutrimentos (DM) y anemia en niños mexicanos. Material y métodos: Se analizó información sérica de 1 382 niños de 1-4 años y de 3 590 niños de 5-11 años participantes de la Ensanut 2018-19. Se categorizó la deficiencia de hierro (DH), B12 (DB12), depleción de vitamina A (DepVA) y anemia. Se emplearon modelos de regresión logística para identificar las características asociadas con la DM. Resultados: La anemia afectó al 28.9 y 19.2%, DH al 10 y 5.1%, DB12 al 5.1y 4.8% y DepVA al 4.7 y 4.3% de los niños preescolares y escolares respectivamente. El 18.5 y 13% de los niños tuvo al menos una DM asociada con anemia. Conclusiones: Los niños más jóvenes tuvieron las prevalencias más altas de DM y anemia. Es necesario el refuerzo en los programas de fortificación y suplementación para evitar las consecuencias a largo plazo asociadas con las DM.

Objective:This was a retrospective study to compare the serum 25-hydroxy vitamin D [25(OH)D], retinol binding protein 4(RBP-4) and other clinical data in type 2 diabetes mellitus (T2DM) patients with or without diabetic nephropathy (DN) and to explore the clinical significance of these indicators in DN.Methods:1946 T2DM patients were enrolled in this study. The T2DM patients were divided to group with diabetic nephropathy (DN group) and without diabetic nephropathy (NDN group). According to the urine albumin to creatinine ratio (UACR), DN patients were further divided into microalbuminuria subgroup (UACR 30~300 mg/g) and massive proteinuria subgroup (UACR> /g). Clinical characteristics including serum 25(OH)D, RBP-4 and other biochemical indicators were collected.Results:Compared with NDN group, DN group showed longer disease duration, older age and higher levels of HbA1c, RBP-4, hs-CRP, TC and TG; 25(OH)D and HDL-C in DN group were lower than those in NDN group ( P<0.05). Within DN group, massive proteinuria subgroup showed higher RBP-4, younger age and lower 25(OH)D and HDL-C than microalbuminuria subgroup ( P<0.05). After adjusted for age, gender and disease duration in DN, partial correlation analysis showed that 25(OH)D is positively correlated with eGFR, and negatively correlated with RBP-4 and UACR ( P<0.05). UACR is positively correlated with RBP-4 and TC, and negatively correlated with eGFR (all P<0.05). eGFR is negatively correlated with RBP-4, TC and UACR (all P<0.05). Multivariate logistic regression showed that disease duration, HbA1c, RBP-4 and hs-CRP are risk factors for DN, and 25(OH)D is the protective factor for DN. Conclusions:Decreased 25(OH)D and increased RBP-4 are associated with increased DN risk in T2DM patients, and also associated with exacerbated albuminuria and deteriorated renal function in DN patients. There is a negative correlation between 25(OH)D and RBP-4 in DN. Therefore, it is necessary to strengthen the monitoring of serum 25(OH)D and RBP-4 and enhance vitamin D supplementation in T2DM patients to prevent the occurrence and delay the progression of diabetic nephropathy.

@#Retinol dehydrogenase 5(RDH5)is an NAD(H)retina-dependent oxidase. As a key enzyme in the visual cycle, it can initiate a series of enzymatic reactions to produce visual pigment, so as to achieve the conversion of photoelectric signal and participate in the formation of retinoic acid, <i>etc</i>. RDH5 mutation can greatly reduce the enzyme activity and even cause severe hereditary retinopathy, such as fundus albipunctatus, retinitis pigmentosa, and retinitis punctate albescens. In this paper, the research progress of RDH5 in visual cycle and hereditary retinal diseases in recent years is reviewed.

@#AIM:To explore the serum retinol binding protein 4(RBP4), inflammatory index neutrophil to lymphocyte ratio(NLR), platelet to lymphocyte ratio(PLR)in patients with type 2 diabetes mellitus(T2DM)after diabetic retinopathy(DR)levels and factors affecting the occurrence of DR in patients. <p>METHODS: The clinical data of 142 patients with T2DM who reached the standard of blood glucose control in our hospital from February 2017 to February 2020 were analyzed retrospectively. According to the results of fundus angiography, the patients were divided into Normal group(<i>n</i>=74), NPDR group(<i>n</i>=36)and PDR group(<i>n</i>=32). Compared the general data and blood test indexes of the three groups, and analyzed the factors affecting the occurrence of DR by multivariate Logistic regression analysis. Constructed a line chart prediction model to predict the occurrence of DR and evaluated its predictive efficiency. <p>RESULTS: The course of DM, serum GH, IGF-I, LDL-C, UA, RBP4, NLR and PLR in PDR group were significantly higher than those in Normal group and NPDR group, while C-P and 2h C-P were significantly lower than those in Normal group and NPDR group. The course of disease >12a, IGF-I >145μg/L, C-P <0.75ng/mL, UA >245ng/mL, RBP4 >54mg/L, NLR >1.8 and PLR >110 were independent risk factors for DR. The line chart model has high degree of differentiation and calibration, and has good prediction efficiency. <p>CONCLUSION: In addition to the common risk factors such as the course of DM, IGF-I, C-P and UA, the increase of RBP4, NLR and PLR is also related to DR, which may be involved in the occurrence and development of DR.

Objective To investigate the application value of Multi-Latex polygranular technique joint detection of kidney injury-related urinary microproteins in noninvasive diagnosis after renal transplantation. Methods Clinical data of 72 recipients undergoing renal transplantation were retrospectively analyzed. According to the level of serum creatinine (Scr), the recipients were divided into normal renal function group (group A, n=14), mild kidney injury (group B, n=37), and severe kidney injury group (group C, n=21). 20 healthy volunteers were selected as the healthy control group (HC group). The contents of urinary retinol binding protein (RBP), microalbumin (mAlb), IgG, transferrin (TRF), α1-microglobulin (MG), and β2-MG of subjects in each group were detected using the Multi-Latex polygranular technique. The correlation between urinary microproteins and Scr, blood urea nitrogen (BUN) was analyzed. The differences of urinary microproteins in each group were compared. And the diagnostic value of single and joint detection of urinary microproteins was evaluated. Results Six kinds of urinary microproteins in HC group and group A were significantly lower than those in group B and group C, and six kinds of urinary microproteins in group B were significantly lower than those in group C (all P < 0.01). Six kinds of urinary microproteins in renal transplant recipients were positively correlated with BUN. RBP, mAlb, α1-MG, and β2-MG were positively correlated with Scr. The correlations were statistically significant (P < 0.001-0.05). The diagnostic value of joint detection of urinary microproteins is better than the detection of single index, among which TRF+mAlb+RBP+α1-MG quadruple detection had the highest diagnostic value. Conclusions Six kinds of urinary microproteins can be used as specific indicators to reflect graft renal function. The polygranular technique can simultaneously detect its contents and achieve noninvasive diagnosis. The diagnosis based on TRF+mAlb+RBP+α1-MG quadruple detection is expected to further improve the noninvasive diagnosis system after renal transplantation.

ABSTRACT

A deficiência de vitamina A e os distúrbios neurológicos em idosos constituem problemas de Saúde Pública que devem ter mais atenção por parte das políticas públicas do idoso institucionalizado, devido à sua gravidade e diversidade de causas. Assim, este estudo objetiva avaliar os fatores de risco associados à deficiência de vitamina A em idosos institucionalizados da cidade de João Pessoa-PB. Trata-se de um estudo transversal e analítico, em que foram selecionados 105 idosos, ≥60 anos, de cinco Instituições de Longa Permanência para Idosos. Avaliaram-se parâmetros sociodemográficos, clínicos, antropométricos, de retinolemia, de consumo de vitamina A, de estresse oxidativo e inflamação. Encontrou-se uma prevalência de deficiência de vitamina A (<1,05 micromol/L) em 30,5% da amostra (32 idosos). Em relação ao consumo de alimentos-fonte de vita- mina A, 68,6% (72 idosos) da amostra consumiam os de origem animal numa frequência maior do que 3x/semana, enquanto que os alimentos de origem vegetal eram consumidos por 64,8% (68 idosos) da amostra, nessa mesma frequência. A capacidade antioxidante total foi quantificada em 10±17 % e o malondialdeído 3,3±1µmol; a alfa glicoproteína ácida em 38±33 mg/dL (p>0,05). Os indivíduos esquizofrênicos possuem 7,75 (p =0,00; IC 95% = 2,56-26,59) vezes mais chances de serem deficientes de retinol sérico, independen- temente de outros fatores de risco. Assim, existe a necessidade de adoção de medidas de saúde pública e um maior controle desses fatores de risco que podem piorar/afetar a qualidade de vida dessa população idosa institucionalizada.(AU)


Vitamin A deficiency and neurological disorders in the elderly are Public Health problems that should be given more attention by public policies for institutionalized elderly people, due to their severity and diversity of causes. Thus, this study aims to assess the risk factors associated with vitamin A deficiency in institutionalized elderly in the city of João Pessoa-PB. This is a cross-sectional and analytical study, in which 105 elderly, ≥60 years old, from five Long Term Care Institutions for the Elderly were selected. Sociodemographic, clinical, anthropometric, retinolemia, vitamin A consumption, oxidative stress and inflammation parameters were evaluated. A prevalence of vitamin A deficiency (<1.05 micromol / L) was found in 30.5% of the sample (32 elderly). Regarding the consumption of vitamin A source foods, 68.6% (72 elderly) of the sample consumed those of animal origin at a frequency greater than 3x/week, while foods of plant origin were consumed by 64.8% (68 elderly) of the sample, at the same frequency. The total antioxidant capacity was quantified at 10 ± 17% and malondialdehyde 3.3 ± 1 µmol; alpha acid glycoprotein at 38 ± 33 mg / dL (p> 0.05). Schizophrenic individuals are 7.75 (p = 0.00; 95% CI = 2.56-26.59) times more likely to be serum retinol deficient, regardless of other risk factors. Thus, there is a need for the adoption of public health measures and greater control of these risk factors that can worsen/affect the quality of life of this institutionalized elderly population.(AU)

Introducción. La deficiencia de vitamina A (DVA) ha sido reconocida como un importante problema de salud pública en países en vías de desarrollo. Los niños en edad preescolar son grupos de riesgo en poblaciones vulnerables. El objetivo fue determinar la prevalencia de DVA y los factores asociados en una muestra de niños de edad preescolar. Material y métodos. Estudio de corte transversal que incluyó a niños de 1 a 6 años beneficiarios de planes sociales, atendidos en centros de atención primaria de la periferia de la ciudad de la Plata, Buenos Aires. Se determinó el contenido de vitamina A midiendo retinol sérico, por cromatografía líquida, y se registraron parámetros antropométricos e ingesta alimentaria. Se utilizó un modelo de regresión logística multinomial para evaluar la asociación entre las variables. Resultados. Se analizaron datos de 624 niños. La media geométrica de retinol fue 23,8 pg/dl (IC 95 %: 23,3-24,3). Las prevalencias de DVA y riesgo de DVA fueron 24,3 % y 57,4 %, respectivamente. Los niveles de retinol fueron significativamente menores en niños varones, con bajo peso y aquellos con bajo consumo (menor del primer tercilo de distribución). El análisis multivariable mostró asociación significativa entre DVA y el sexo masculino (OR: 1,93; IC 95 %: 1,15-3,24) y con el bajo consumo (OR: 1,48; IC 95 %: 1,15-2,62). Conclusión. La prevalencia de DVA hallada (24,3 %) constituye un importante problema de salud pública en esta población. Los factores asociados a dicha deficiencia fueron el sexo masculino y el bajo consumo.


Introduction. Vitamin A deficiency (VAD) has been recognized as an important public health problem in developing countries. Preschoolers account for risk groups within vulnerable populations. The objective of this study was to determine the prevalence of VAD and associated factors in a sample of preschoolers. Material and methods. Cross-sectional study with children aged 1-6 years receiving social assistance and seen at primary health care centers in the outskirts of the city of La Plata, Buenos Aires. Vitamin A levels were determined by measuring serum retinol with a liquid chromatography; anthropometric parameters and dietary intake were recorded. A multinomial logistic regression model was used to assess the association among outcome measures. Results. Data from 624 children were analyzed. The geometric mean of retinol was 23.8 pg/dL (95 % CI: 23.3-24.3). The prevalence of VAD and the risk for VAD were 24.3 % and 57.4 %, respectively. Retinol levels were significantly lower among boys, low-weight children, and those with low intake (below the first tertile of distribution). The multivariate analysis showed a significant association between VAD and male sex (odds ratio: 1.93; 95 % CI: 1.15-3.24) and between VAD and low intake (odds ratio: 1.48; 95 % CI: 1.15-2.62). Conclusion. The prevalence of VAD (24.3 %) is a major public health problem in this population. VAD-associated factors were male sex and low vitamin A intake.

ABSTRACT

Mitochondrion-localized retinol dehydrogenase 13 (Rdh13) is a short-chain dehydrogenase/reductase involved in vitamin A metabolism in both humans and mice. We previously generated Rdh13 knockout mice and showed that Rdh13 deficiency causes severe acute retinal light damage. In this study, considering that Rdh13 is highly expressed in mouse liver, we further evaluated the potential effect of Rdh13 on liver injury induced by carbon tetrachloride (CCl). Although Rdh13 deficiency showed no significant effect on liver histology and physiological functions under regular culture, the Rdh13 mice displayed an attenuated response to CCl-induced liver injury. Their livers also exhibited less histological changes and contained lower levels of liver-related metabolism enzymes compared with the livers of wild-type (WT) mice. Furthermore, the Rdh13 mice had Rdh13 deficiency and thus their liver cells were protected from apoptosis, and the quantity of their proliferative cells became lower than that in WTafter CCl exposure. The ablation of Rdh13 gene decreased the expression levels of thyroid hormone-inducible nuclear protein 14 (Spot14) and cytochrome P450 (Cyp2e1) in the liver, especially after CCl treatment for 48 h. These data suggested that the alleviated liver damage induced by CCl in Rdh13 mice was caused by Cyp2e1 enzymes, which promoted reductive CCl metabolism by altering the status of thyroxine metabolism. This result further implicated Rdh13 as a potential drug target in preventing chemically induced liver injury.

ABSTRACT

Objective To investigate the value of serum amyloid A (SAA) and retinol binding protein (RBP) in diagnosis of early type 2 diabetic kidney disease (DKD).Methods A total of 182 type 2 diabetic patients hospitalized in department of Endocrinology and Nephrology of Lianyungang Second People's Hospital from January to December 2017 were randomly collected as subjects.According to urinary albumincreatinine ratio (UACR),all subjects were divided into three groups of normal albuminuria group (NA,60 cases),microalbuminuria group (MA,63 cases) and clinical proteinuria (CP,59 cases).In the same period,60 healthy persons were selected as normal control (NC).The levels of serum SAA and RBP were detected by automatic biochemical analyzer.Receiver operating curve (ROC) was used to analysis the diagnostic efficiency of DKD and the risk factors of DKD were further estimated.Results The levels of SAA were (6.88±2.82) and (37.21±20.58) mg/L in control group and case group.And levels of serum SAA in the NA group,MA group and CP group were (16.33±5.98),(40.97± 15.62),(54.43±22.91) mg/L respectively.The levels of RBP were (37.56± 10.51) and (69.26±21.23) mg/L in control group and case group.And levels of RBP in the NA group,MA group and CP group were (52.66 ± 14.31),(69.66 ± ± 15.52),(85.70± 19.51) mg/L respectively.The concentrations of serum SAA and RBP in type 2 diabetic patients were significantly higher than those normal controls (t =10.36,P<0.05;t =11.11,P<0.05) and increased with DKD progression (F =83.6,P< 0.05;F =59.2,P< 0.05).Diagnostic sensitivity of serumSAA,RBP in DKD was 81.8% and 84.1% respectively.Diagnostic sensitivity of combined detection was 90.9%.Regression analysis showed that SAA,RBP and UACR were independent risk factors for DKD (OR (95% CI) =1.391 (1.068-1.812),1.212 (1.085-1.353),1.148 (1.038-1.270);all P <0.05).Conclusion Both of serum SAA and RBP were significantly elevated in diabetic patients with renal injury so that they had great value in early diagnosis of DKD.

Objective To explore the relationship between serum retinol binding protein 4 and serum Lipasin levels and vascular complications in gestational diabetes mellitus(GDM).Methods From Jan.2016 to Jan.2018,80 pregnant women with gestational diabetes diagnosed as GDM in Wenzhou Central Hospital of Zhejiang Province were selected as the study group.They were divided into two groups according to whether they had vascular lesions.Group A included patients with gestational diabetes mellitus complicated with vascular complications and group B included patients without vascular complications.Forty healthy pregnant women were selected as the control group(group C).The levels of fasting plasma glucose (FPG),fasting serum insulin (FINS),homeostatic model assessment of insulin resistance (HOMA-IR),serum retinol binding protein 4 (RBP4) and serum Lipasin were compared among three groups of pregnant women.The vascular complications of GDM were analyzed.Results The levels of FPG,FINS and HOMA-IR in gestational diabetes mellitus pregnant women were higher than those in healthy pregnant women (P<0.05),and those in group A were higher than those in group B (P<0.05).The levels of RBP4 and Lipasin in serum of pregnant women with GDM were higher than those of healthy pregnant women (P<0.05),and those of group A were higher than those of group B (P<0.05).Spearman univariate correlation analysis showed that serum RBP4 levels were positively correlated with FPG and FINS(P<0.05),and serum Lipasin levels were positively correlated with FPG and FINS (P<0.05).Logistic regression analysis showed that the levels of FPG,FINS,RBP4 and Lipasin increased,which were independent risk factors for diabetic retinopathy (P<0.05).Conclusion In GDM with vascular complications,the serum RBP4 and Lipasin levels are higher,which are independent risk factors for vascular complications in GDM,and may participate in the occurrence and development of vascular complications in gestational diabetes mellitus.
